April 9, 2006, at the Blast Furnace at AK Steel Middletown Works,
precautions and training
were inadequate to ensure that water did not accumulate in the East #1
slag pit.  Among other
methods, one feasible and acceptable abatement method to correct this
hazard is to develop, train
on and enforce a procedure to ensure that water does not accumulate in the
slag pits.
