were causing or likely to cause death
or serious physical harm to employees in that employees were exposed to
falling and/or being thrown from a
forklift:

and at times prior thereto, the employer did not instruct and/or enforce
the utilization of seat belts by
operators of counter balanced, center controlled high lift trucks with
sit-down non-elevating operator
positions, which exposed them to hazards including but not limited to
being thrown from the lift.
Among other methods, several feasible and acceptable methods to correct
this hazard is by complying with the
manufacturer's operation guide regarding use of the manufacturer supplied
seatbelts and/or QNWI/ITSDF

Restraint Systems
