Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to crushing and struck-by injuries due to wire rope guyline failure: (a)Bogner Sale: On or about May 31, 2007, and at times prior thereto, the eye for a wire rope guyline for the Thunderbird skyline yarder had no less than 7 broken and crushed wires in one strand of one lay. (b)Bogner Sale: On or about May 31, 2007, and at times prior thereto, the steel shackle used to choke a stump for attaching the center guyline to the Thunderbird skyline yarder used a severely worn and makeshift pin that was the wrong size for the shackle. FEASIBLE AND ACCEPTABLE ABATEMENT METHODS, AMONG OTHERS, TO CORRECT THIS HAZARD include ensuring guylines and rigging hardware are appropriate and in good condition by: - Inspecting wire rope and hardware for damage and wear according to the manufacturer or industry recognized standards including; Wire Rope Users Manual and ANSI B30.9-1996 Safety Standard for Cableways, Cranes, Derricks, Hoists, Hooks, Jacks and Slings ; - Removing from service damaged wire rope when the capacity has been reduced, wires have been crushed/distorted, or there are 10 or more broken wires randomly distributed in one rope lay or there are 5 or more broken wires in one strand of one lay, as described in ANSI B30.9-1996 Safety Standard for Cableways, Cranes, Derricks, Hoists, Hooks, Jacks and Slings; - Using thimbles or equivalent methods to prevent damage in wire rope loops/eyes; and - Using the proper size pin designed by the manufacturer to be used with the shackle, and securing the pin using cotter-keys, molles, or the equivalent per the Idaho Minimum Safety Standards and Practices for Logging - IDAPA 17, Title 08. NOTE: Abatement certification IS required for this item.
