Section 6(a) of the Puerto Rico Occupational Safety and Health Act:  The
employer did not
furnish employment and a place of employment which were free from

that were causing or likely to cause death or physical harm to employees

were exposed to:

El patrono no
proveyo empleo y un sitio de empleo libre de riesgos reconocidos que esten
causando o que
puedan causar muerte o dano fisico a los empleados, por cuanto empleados
estaban expuestos
a:
a.  Receive serious injuries such as death due to fall automotive lift
over employee.
Were interview and documented that employee was lifted cars with defective
automotive lifts, serial 501192, model NDL 7; and another serial 56179/05,
model
MX-10ACX in mechanical workshop. Among other methods, one feasible and
acceptable abatement method to correct this hazard is perform the
preventive
maintenance according to ANSI/ALI ALOIM 2000, sec 6.1.1.
a.  Recibir lesiones serias tales como muerte debido a caida de pino de
automoviles
sobre empleado.  Fue entrevistado y documentado que empleado estaba
levantando
carros con pinos defectuosos, serie 501192, modelo NDL 7; y otro serie
56179/05,
modelo MX-10ACX en taller de mecanica. Entre otros metodos, un metodo
aceptable
y posible de corregir este riesgo es realizar el mantenimiento preventivo
de acuerdo a
ANSI/ALI ALOIM-2000, sec 6.11.

a.  Receive serious injuries due to explosion tires or pressure vessel.
The employee
was using the pressure vessel 200psi, ID #210563, with pressure gauge
broken, to
inflate car tires in the mechanical workshop. Among other methods, one
feasible and
acceptable abatement method to correct this hazard is put a new pressure
gauge and
inspect the pressure vessel.
a.  Recibir lesiones serias debido a explosion de gomas o recipiente a
presion. El
empleado estaba usando el recipiente a presion de 200 psi, ID #210563, con
el reloj
medidor de presion roto, para inflar gomas de auto en el taller de
mecanica. Entre
otros metodos, un metodo aceptable y posible de corregir este riesgo es
ponerle
un
reloj de presion nuevo e inspeccionar el recipiente de presion.

level. Was documented through interview in the storage area, that
employees
were rising
over mezzanine 6 feet 7 inches hight to look for merchandise with folding
aluminum
metal ladder 6 feet long. Among other methods, one feasible and acceptable
abatement
method to correct this hazard is to provided the correct ladder with the
appropiate hight.
a. Recibir serias lesiones debido a caida de entrepiso de 6 pies 7 pulgs.
de alto sobre el
nivel maa bajo. Fue documentado por medio de entrevista en el area de
almacen, que
empleados estaban subiendo sobre entrepiso de 6 pies 7 pulgs. de alto para
buscar
mercancia con una escalera de aluminio plegable de 6 pies largo.  Entre
otros metodos,
un metodo aceptable y posible de corregir este riesgo es proveer la
escalera correcta con
la altura apropiada.

a.  Employer Autocentro del Sur Inc. did not have a written protocol to
handle
domestic violence in the workplace, as required by the Act for the
implementation of
a Protocol to Handle Domestic Violence Situations in the Workplace or
Place of
Employment, Act No.217 of 2006 (for private and public employers, except
municipalities).
Hazard:  Fatalities, injuries, and illnesses related to domestic violence
in the
workplace.
Method to abate the hazard:  Develop a written protocol to handle domestic
violence
in the workplace and provided training to all employees. Information is
available at
the Women's Advocate Office. A guide for Employers: Domestic Violence in
the
Workplace and others references provided during the inspection.
a.  El patrono Autocentro del Sur Inc. no tenia un protocolo escrito para
manejar
violencia domestica en el lugar de trabajo, como es requerido por la Ley
para la
Implementacion de un protocolo para manejar Situaciones de Violencia
Domestica en
el Lugar de Trabajo, Ley No. 217 de 2006 (para patronos privados y
publicos,
excepto municipios).
Riesgo:  Fatalidades, lesiones, y enfermedades relacionadas con violencia
domestica
en el lugar de trabajo.
Metodo para corregir el riesgo:  Desarrollar un protocolo escrito para
manejar
violencia domestica en el lugar de trabajo y proveer adiestramiento a
todos los
empleados. La informacion esta disponible en la Oficina de la Procuradora
de las
Mujeres, Una Guia para Patronos: Violencia Domestica en el Lugar de
Trabajo y
otras referencias provistas durante la inspeccion.
