Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: At the worksite, for job operations in the service hanger,the employer had not conducted an inspection of manually operated chain hoist by a designated person. Employees used manually operated chain hoist for purposes such as but not limited to lifting, lowering and supporting jet aircraft engines during engine replacement. The cable hoist had not been inspected in accordance with recognized industry standards, such as American Society of Mechanical Engineers (ASME) standard B30.21-2005, sections 21-1.31(a) and (b). The cable hoist used in the engine replacement operations had not been inspected and/or were not inspected on a periodic basis. This condition exposed employees to the hazard of being struck by falling objects in the event of hoist failure.

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: At the facility, the Caterpillar forklift was not equipped with a seatbelt. The forklift was used for job tasks such as but not limited to loading and unloading supplies. Employees operated the forklift in and around the facility and used the forklift to retrieve supplies from an adjacent building. This condition exposed employees to the hazard of being struck by the forklift in the event of a tip over accident.

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: At the facility, employees were provided with and/or utilized personal fall arrest systems that did not conform to established and recognized standards and/or criteria. Specifically, the vertical lifelines used at the facility that were suspended from the ceiling of the service hanger were not protected against abrasion. The ropes used as lifelines were wrapped directly around metal beams, subjecting them to being cut and/or abraded. The method of attachment could result in failure of the lines and exposed employees to the hazard of falling from an elevation.
