40.1-51.1(a), Code of Virginia: The employer did not furnish employment
and a place of employment which
were free from recognized hazards that were causing or likely to cause
death or serious physical harm to
employees in that: At this site in the meat department on 2/17/2006 an
employee was performing meat cutting
operations with a band saw and amputated the distal tip of his middle
finger. The employee was standing on a
piece of card board while performing cutting operations. The floor was wet
from cutting meats prior to the
accident. The cardboard displaced causing the employee's back foot to slip
and the employee's hand went
forward  into the bandsaw resulting in an amputation of his distal tip of
his middle finger.
" ONE FEASIBLE AND ACCEPTABLE ABATEMENT METHOD, AMONG OTHERS TO CORRECT
THIS HAZARD IS FOLLOW COMPANY PROCEDURES WHICH STATES: SAFETY REGULATIONS
MUST BE FOLLOWED WHEN OPERATING A BAND SAW;  #2 ON SAFETY REGULATIONS SIGN
STATES: PLEASE MAKE SURE THE FLOOR AROUND THE BAND SAW IS DRY.  RUBBER MATS
MUST ALWAYS BE USED AT THE BAND SAW.

finger. The employee was standing on a
piece of card board while performing cutting operations. The floor was wet
from cutting meats prior to the
accident. The cardboard displaced causing the employee's back foot to slip
and the employee's hand went
forward  into the bandsaw resulting in an amputation of his distal tip of
his middle finger. At the time of the
accident the employee was cutting porter house stakes on the band saw and
was not using the push plate to hold
the meat that he was cutting. The blade was not fully guarded when the
meat was being cut by the employee.
The employee was holding the meat by hand near the blade. The meat that
the employee was cutting was larger
than the carriage of the machine.  The operations manual for the band saw
was not on the premises.
" ONE FEASIBLE AND ACCEPTABLE ABATEMENT METHOD, AMONG OTHERS TO CORRECT
THIS HAZARD IS FOLLOW MANUFACTURES OPERATION PROCEDURES IN THE
INSTRUCTIONS MANUAL FOR MODEL 6614 MEAT SAW WHICH STATES: SAFETY DEVICES
INCORPORATED IN THIS SAW MUST BE IN THEIR CORRECT OPERATING POSITIONS
ANYTIME THE SAW IS IN USE. SAFETY FEATURES: PUSHER PLATE; ALWAYS KEEP HANDS
WELL BACK FROM THE BLADE AND MAINTAIN CONTROL OF THE PRODUCT. USE PUSHER
PLATE AS DESCRIBED (PAGE 10) AND IT WILL BE UNNECESSARY TO HOLD YOUR HAND
NEAR THE CUTTING EDGE OF THE SAW BLADE. IN ADDITION, UPPER GUIDE AND GUARD
ASSEMBLY:  BEFORE TURNING THE SAW ON, RAISE OR LOWER THE UPPER GUIDE AND
GUARD ASSEMBLY BY GRASPING THE HAND KNOB AND SLIDING UP OR DOWN SO THAT
THE CUTTING ZONE IS ONLY HIGH AS NECESSARY FOR THE PIECE BEING CUT.
