Title 40.1-51.1(a), Code of Virginia: Employer did not furnish employment, and a place of employment which were free of recognized hazards that were causing, or likely to cause death or serious physical harm to employees in that: Employee(s) was exposed to fall hazards while performing job related duties when employee(s) was permitted to work from a fabricated metal job box lifted by the forks of a TEREX TH1056C forklift. The TEREX TH1056C forklift should not be used to elevate personnel, as it is not designed for this purpose. One feasible, and acceptable abatement method among others to correct this hazard is to comply with the sign posted in the operators compartment, and on front hydraulic cylinder which both read: DANGER Never equip machine with any form of personnel platform or allow personnel to ride on attachment.,
