Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not furnish employment
and a place of employment which were free from recognized hazards that
were causing or likely to cause
death or serious physical harm to employees in that employees were exposed
to:
(a) Location - 600 Unicorn Park, Woburn, MA:
On or about 06/22/06, an employee washing windows while suspended from a
rolling outrigger beam
suspension unit was exposed to crushing hazards, struck-by hazards and
fall hazards of up to 60 feet
while:
1) The Roof Roller was not tied back to an anchorage point on the
building, as per the manufacturer's
instructions and ANSI/IWCA I-14.1-2001.
2) Anchorage points were not supplied for the roof roller within a 15
degree angle for
tiebacks.
3) The required hazard assessment was not performed prior to the start of
work.
4) The manufacturer's instructions were not provided to the employees
operating the roof roller.
5) The wheels on the roof roller were not locked in place while an
employee was suspended from the
roof roller.
Among other methods, feasible and acceptable abatement methods to correct
this hazards include but
not limited to:
1) Assign a competent person to each job site and ensure that (s)he
inspects the set-up, rigging,
anchorage and all aspects of the roof roller and or other outrigger
suspension equipment prior to and
during each use. 2) Ensure that the outrigger beam suspension unit such as
the rolling roof  rig is
anchored or tied back to a secure anchorage point on the building at all
times when in use as required
by ANSI/IWCA STD I-14.1-2001 and the manufacturer's instructions. 3) When
scheduling
work to be
performed, ensure that a competent person conducts a full and complete
on-site inspection and hazard
assessment for rigging, anchorage and operation of the outrigger
suspension units to be used in the
window washing operations.

(a) Location - 600 Unicorn Park, Woburn, MA:
On or about 06/22/06, employees were exposed to a serious fall hazard of
up to 60 feet while working
at the roof edge without the use of personal fall protection equipment.
(b) Location - 600 Unicorn Park, Woburn, MA:
On or about 06/22/06, employees were exposed to a serious fall hazard of
up to 60 feet while working
from a rope descent system connected to a rolling roof system without
independent anchorage points to
the roof for their personal fall protection equipment.
Among other means, feasible means of abatement would be to:
1) Assign a competent person to each job site and ensure that (s)he
inspects the set-up, rigging,
anchorage and all aspects of the outrigger suspension equipment prior to
and during each use. 2)
Ensure that the outrigger beam suspension unit such as the rope descent
equipment is anchored or tied
back to a secure independent anchorage point on the building at all times
when in use as required by
ANSI/IWCA STD I-14.1-2001 and the manufacturer's instructions. 3) When
scheduling work to be
performed, ensure that a competent person conducts a full and complete
on-site inspection and hazard
assessment for rigging, anchorage and operation of the outrigger
suspension units to be used in the
window washing operations. 4) Ensure employees are supplied with the
proper fall protection
equipment while on the roof and suspended by the rope descent system.
The Crystal Bright Facility Services Company was previously cited for a
violation
of this Occupational
Safety and Health standard or its equivalent standard 29CFR1904.5(a)(1),
which was contained in
