Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to potential crushing or struck-by hazards: In the following instances, the employer did not comply with the mandatory language in accordance to ANSI R15.06 - 1999; Industrial Robots and Robot Systems - Safety Requirements, Clause 5 and ANSI B11.19-1990 Performance Criteria for the Design, Construction, Care and Operation, Clauses 3, 4, & 6: a) Production Area - The employer did not have functional safety interlock switches on the robotics automation guard doors to #1, 2 & 7 molding machines. b) Production Area - The employer did ensure functional safety interlock switches to the sliding doors for the operating side of molding machines #1 - 24. Abatement Note: Among other methods, one feasible and acceptable method to correct the following hazards is to: Instances a & b: Ensure proper safeguarding is installed, including but not limited to, functional interlocks that ensure that the robot does not continue to cycle. Train employees who will be deemed qualified to inspect for defects prior to the start of each shift and maintain the interlocks in functional order.
