a.On or about June 11, 2007, the front barrel of a control gun used to
water blast
the interior of a hot well tank was modified from the original
manufactured length
of 48 inches to a length of 28-1/8 inches, exposing an employee to a water-
injection/struck-by hazard associated with the use of a high pressure
9,500 p.s.i.
control gun.
Among other methods, one reasonable means of abatement would be:
1.  To assure that the length of control gun assemblies are maintained
according to the
manufacturer's guidelines.
