Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to hazards associated with rotating parts on horizontal
lathes:
At this location on or about 06/18/2006, and at times prior thereto,
employees
involved in a machining operation were using horizontal lathes and the
following
safeguards were not implemented:
a.The employer did not ensure reasonable tasks and associated hazards were
identified concerning the operation of manual turning machines servicing
parts with protrusions.  The employer had not developed a machine and/or
job specific hazard analysis to prevent a mishap.
b.The employer did not develop a control strategy designed to eliminate or
reduce identified hazards such as rotating parts with protrusions.
c.The employer did not ensure correct and safe work practices were being
followed in that employees were permitted to wear clothing capable of
becoming entangled with the rotating parts with protrusions.
d.The employer did not assign a supervisor to ensure operating procedures
were within the guidelines described in the suppliers operation manual,
i.e., there was no supervisor on duty to ensure employees did not:
1.Wear clothing that might become entangled by rotating parts with
protrusions.
2.Reach into the work area of the machine while the spindle is
turning.
One feasible means of abatement is to ensure that all employees, both
management and shop employees are trained in and comply with the above
cited
standards to reduce the hazards of operating a manual turning machine.
The above items were cited from the American National Standards Institute
manual B11.6-2001 titled "Safety Requirements for Manual Turning Machines
with or without Automatic Control".
