industrial truck, Toyota, model #42-6FGCU15, serial #66414, 6,210 lbs.
capacity, when the
installed and attached seat belt was not fastened and in use.
Among others, one feasible and acceptable abatement method, among others,
to correct this
hazard is to develop, establish and/or update written procedures on proper
equipment usage,
train and/or retrain employees on applicable and acceptable established
procedures, prohibit
employees from performing and/or attempting to perform inappropriate and
unsafe procedures,
and enforce established and/or applicable policies and procedures for
equipment usage.
