(a)On or about 13 June 2006, the operator of the Skytrack and the
Pettybone All-
terrain forklift was not wearing or using the seat belt while operating the
equipment.
Among other methods, one feasible and acceptable abatement method to
correct this hazard is
to instruct each employee during initial training and safety meetings to
use the operator restraint
system (seat belts) in accordance with Pettybone's and SkyTrk's
Owners/Operators Manaul,
Model 8042, conduct periodic monitoring inspections of rough terrain
forklift drivers, and
correct deficiencies immediately.
