Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to falls from being lifted on the elevated forks of an
industrial fork lift truck.
(a)  Location - Bay State Pallet Co., Inc., Bldg. 3A
On or about 6/26/06, an employee on wooden pallet with a job made tool
bench was
lifted 14 feet high on the elevated forks of an industrial fork lift truck.
Among other methods, feasible and acceptable means of abatement to correct
this hazard
as follows:  1)  With approval of the fork lift truck manufacturer, the
purchase and use
of a proper safety platform meeting specifications of the ANSI B56.1-1975
standard.  2)
With the approval of the fork lift truck manufacturer, the in-house
fabrication and use
of a proper safety platform certified by a professional engineer and
meeting
specifications
of the ANSI B56.1-1975 standard.5
