Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of being struck by falling suction pressure rolls and blind drill roll due to the failure of the manual chain hoists and the below the hook devices due to: (a)The two 10 ton manual chain hoists used to remove 32,987 pound suction pressure rolls abd the 24,000 pound blind drill rolls on paper machines #1 and #2 during tandem lifts were not sufficiently cleaned prior to lift operations. (b)Hoist operators and hoist operations superisor involved in manual hoist lifts performed every 3 months for the suction pressure roll and every 6 months for the blind drill roll were not adequately trained on how to perfrom multiple hoist lifts. (c)The two hand-operated 10-ton hoist were being operated by two operators at the same time to lift the single chain attached to a 32,000 pound sunction pressure role on paper machine number two. Among other methods, one feasible and acceptable abatement method to correct the hazards listed above is to follow the requirements of ASME B30.16-2003 for Overhead Hoists for Below-the-Hook lifting devices. The Employer agrees to ensure that the above hoisting operations shall be conducted such theat suction pressure rolls and blind drill rolls are balanced to prevent overloading of hoists during multiple hoist lifts. The Employer further agrees to provide additional traininging for hoist operations supervisors on safe rigging and lifting methods. The abatement date shall be modfied to January 15, 2007.
