is not recommended and or approved by the manufacturer.

to operate and lift a Poly-John, Model #PL01-1000 30 feet to the third
floor using
lifting straps placed around the forks of a JLG 1044C-54 Series II Rough
Terrain
Forklift when this practice is not recommended and or approved by the
manufacturer.
Condition observed by CSHO on or about 03/27/07.
ONE FEASIBLE AND ACCEPTABLE METHOD TO CORRECT THIS HAZARD,
AMONG OTHER MEANS, WOULD BE FOR THE CONTRACTOR TO PURCHASE
AND OR RENT THE APPROPRIATE ATTACHMENTS, I.E. TRUSS BOOM OR
CRANE HOOK TO REPLACE EXISTING FORKS. EITHER OF THESE TWO
ATTACHMENTS ARE BETTER SUITED AND APPROVED FOR THIS TYPE OF
WORK.
