Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to fall hazards in that the fall protection anchorage system used by employees was not properly counterbalanced: (a)Jobsite: Employees placed small narrow pieces of lead weights into the counter balance basket of the rolling fall protection anchorage system. These pieces of lead were narrow enough that they could unexpectedly fall out of the basket, reducing the capacity of the anchorage system. Among other methods, one feasible and acceptable abatement method to correct this hazard would be the use of the steel plates supplied by the manufacturer for this device.
