Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of being struck or crushed by moving parts of a machine: a. On or about 9/15/06 at the Eastwood Village site, Birmingham, AL -employees were exposed to a potential crushing hazard when they entered the crushing chamber to remove a jam of oversized material without implementing lockout/tagout procedures. Among other methods, one feasible and acceptable method to correct this hazard is to disconnect and lock out the power source(s) before attempting to service, clean or un-jam equipment as recommended by the manufacturer.
