Section 5(a)(1) of the Occupational safety and Health Act of 1970: the employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees: Yard area - On or about July 31, 2006, an employee performing service/adjustments on the bucket attachment mechanism of the Mustang 2070 Skid Steer Loader was exposed to struck by/ caught between hazards due to the fact that the equipment's safety lockout system was inoperative. Feasible and acceptable abatement methods include, but are not limited to, the following: Ensure the safety lockout system is operational in accordance with the Mustang Skid Steer Loader Operator's Handbook. Adherence would prevent the operator from exiting the machine with the engine left running.
