Title 40.1-51.1(a), Code of Virginia: Employer did not furnish employment, and a place of employment which were free of recognized hazards that were causing, or likely to cause death or serious physical harm to employees in that: Employee(s) was exposed to fall hazards while performing job related duties when employee(s) was permitted to work from a wooden pallet lifted by the forks of a Gradall G6-42A rough terrain forklift. The Gradall G6-42A forklift should not be used to elevate personnel, unless a Gradall manufactured personnel work platform is used. One feasible, and acceptable abatement method among others to correct this hazard is to comply with the placard attached to machine that reads: Warning - When lifting personnel use only a Gradall manufactured personnel work platform.

Title 40.1-51.1(a), Code of Virginia: Employer did not furnish employment, and a place of employment which were free of recognized hazards that were causing, or likely to cause death or serious physical harm to employees in that: Employee(s) was exposed to fall hazards while performing various job related duties when employee(s) worked from wooden pallet, lifted by the forks of a Gradall G6-42A forklift. The operators manual was not present with the machine. One feasible, and acceptable abatement method among others to correct this hazard is to comply with sign posted on side of machine, and visible when mounting and dismounting the machine which reads: Warning- Read and understand personnel work platform user's manual before occupying personnel work platform.
