Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not furnish employment
and a place of employment which were free from recognized hazards that
were causing or likely to cause
death or serious physical harm to employees in that employees were exposed
to crushing hazards:
On or about 09/08/2006 an employee unloading an aerial lift from a flatbed
trailer was exposed to crushing
injuries. The chains used to load and unload the aerial lift were not
rated for the weight of the aerial lift. The
chain being used to load and unload the lift showed signs of damage.
Among other methods, one feasible and acceptable abatement method to
correct this hazard is to use the
proper size rigging chain capable of holding the weight of the equipment
being loaded and unloaded.
Inspect rigging prior to each use to ensure chains have not been damaged,
when damaged rigging is found,
take it out of service.
