were causing or likely to cause
death or serious physical harm to employees in that employees were exposed
to impact hazards:
Shop:  On, or about, June 28, 2007, a mismatched tire and rim exploded
while being inflated on a tire
changing machine. An employee had assembled a sixteen inch tire onto a
sixteen and a half inch rim.
Among other methods, one feasible and acceptable abatement method to
correct this hazard is to ensure that
all employees who service tires have been trained in properly matching
tires and rims.
