Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck-by hazards: (a) Location - Pike School roof: On or about 8/8/06, an employee was exposed to serious struck-by hazards while walking/working in close proximity to an improperly used roof roll carrier. Among other methods, one feasible and acceptable means to correct this hazard would be: Follow and ensure the safety precautions, job set up and operating instructions listed in the Garlock E-Z Roll Carrier Operator's Instruction Manual, revised May, 1998. Ensure that all users are properly trained in the safe use of the roll carrier.
