Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards
that were causing or likely to cause death or serious physical harm to
employees in that
employees were exposed to amputation hazards:
a) Tool Room- Provisions were not made to ensure that the Jet Equipment &
Tools horizontal
band saw, Serial #5020397, with a device to prevent the saw's motor from
automatically
restarting after power was restored.  A magnetic restart type power switch
was not provided as
prescribed by the American National Standards, ANSI B11.10-2003.
Among other methods, one feasible and acceptable method to correct this
hazard is to include,
but not be limited to the following:
(1) Provide a magnetic restarter or similar device as required by American
national Standards
Institute (ANSI) B11.10-2003 Section 6.11, that in the event of an
interruption in the electrical
supply would prevent the saw from restarting automatically when power is
restored.
In accordance with the requirements of 29 CFR 1903.19(d), abatement
certification is
required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION
WORKSHEET), and in addition, documentation demonstrating that abatement is
complete
must be included with your certification. This documentation may include,
but is not
limited to, evidence of the purchase or repair of the equipment,
photographic or video
evidence of abatement, or other written records.
