cause death or serious
physical harm to employees in that employees were exposed to the hazard of
falling and being
crushed by a forklift:
On or about 10/20/06, three employees were exposed to the hazards of
falling from fork lifts
and being crushed by this type of equipment, while operating powered
industrial trucks without
wearing restraining devices such as, but not limited to seat belts.
Among other methods of abatement, one feasible and acceptable method to
correct this hazard
is to prohibit employees from operating this type of equipment without
wearing seat belts.
PLEASE NOTE: DOCUMENTATION AND CERTIFICATION OF ABATEMENT IS
REQUIRED FOR THIS VIOLATION.
