hazard.
On or about 11/1/2006, at 5500 State Road 7, an employee operating a
Skytrak all-terrain
forklift was exposed to a crushed-by hazard if the lift were to tip over.
One feasible and acceptable method for abatement is to instruct the
employee to always wear a
seatbelt while operating the forklift.

REQUIRED FOR THIS VIOLATION.
