Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish to each of his employees a place of employment which was free from
recognized hazards
that were causing or likely to cause death or serious physical harm to
employees in that
employees were exposed to the hazard of:
For the period of time up to and including 18 Aug 06, the employer did not
ensure that
the back-up alarm on the Traverse Lift Rough Terrain Fork Lift was
operating properly.
When backing-up, the back-up alarm on the equipment could not be heard over
surrounding noise at the site and in the vicinity. This exposed employees
to the hazards
and injuries related to being struck by the equipment.
Among other methods, one feasible and acceptable abatement method to
correct this
hazard would be to:
Ensure that the rough terrain forklift is properly maintained including
insuring that the
back-up alarm is working properly and can be heard over the surrounding
noise levels.
