Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish to each of his employees a place of employment which was free from
recognized hazards
that were causing or likely to cause death or serious physical harm to
employees in that
employees were exposed to: the hazard of an employee falling from the work
platform while
sitting on the top railing and the hazard of the Sky Trak Model 6036 Rough
Terrain Forklift
overturning while employees are working from a twenty-five (25) foot long
work platform. The
work platform extended a total of two-hundred and two (202) inches beyond
the
wheelbase of
the forklift (one-hundred and one (101)) inches per side.
One feasible method of correcting this hazard is to implement and follow
the
American Society of Mechanical Engineers (ASME) B56.6-2002 (Safety Standard
for Rough Terrain Forklift Trucks) Section 5.15; Elevating Personnel:
5.15.1(u):
Make certain that personnel maintain firm footing on the platform floor
unless
secured by body belt and lanyard. ASME B56.6-2002 Section 8.24; Platforms
for
Elevating Personnel: 8.24.1(b): Properly construct the work platform so
that it
is no more that a total of (20) inches (10 inches on each side) wider than
the
wheelbase of the lift.
