Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to being struck by falling material: (a) Matthew Hole # 27: On or about 03/33/07 employees were hoisting pipe using the Stabber hoist in which several cable strands were broken, the cable was bird caged and the hoist line was unraveling at the Crosby clamps. Among other methods, one feasible and acceptable method of abatement is to conduct frequent and periodic inspections of the rope by the operator or the designated person at the start of each shift in accordance with the American National Standard ASME B30.16-2003 Overhead Hoists.
