Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the condition(s) listed below: Basic Energy Services, Rig 1364, Mollie 'B' Well #2, Tarzan, TX. For the period of time up to and including 11/14/06, the means of Auxiliary Escape from the derrick (Geronimo Line) was installed as per manufacturer's instructions in that the escape line did not have enough tension and was dragging on the ground for 20 feet, only two clamps were used to secure the end of the cable, the cable was looped loosely around one stake. One feasible abatement method would include ensuring the Auxiliary Escape is installed according to manufacturer's instructions.
