Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to multiple head fractures and possible death: On or before 7/28/06, employees who do bar changes/set-up on a CNC machine were not adequately trained. One feasible method of abatement is to instruct the operator in the proper method of operation for each production set-up before the machine is placed into production; and require that all operators demonstrate their knowledge of the machine, per ANSI B11.8 - 2001.
