SCRR 71-112A: Failed to furnish a place of employment which is free of
recognized hazards
which may cause death or serious physical harm to his employees and comply
with this
regulation and other occupational safety and health rules and regulations
promulgated under
Chapter 15 of Title 41, Code of Laws, State of South Carolina, 1976, as
amended, as
follows:
A.  Employer knew or should have known that employees working in the
boiler room were
exposed to the hazard of receiving burns with the safety relief valve
venting within the boiler
room.  A feasible and useful method to correct this hazard, among others,
is to have the
safety relief valves vent to the atmosphere.
B.  Employer knew or should have known that employees working in the
boiler room were
exposed to the hazard of being struck by flying objects from the boiler
exploding.  A feasible
and useful method to correct this hazard, among others, is to have formal
periodic
maintenance and testing conducted on the burn management (train) system.
C.  Employer knew or should have known that employees working in the
boiler room or on
boiler were exposed to the hazard of possible boiler explosion by over
pressure and/or over
temperature in the boiler.  A feasible and useful method to correct this
hazard, among
others, is to have a data plate affixed to the boiler for viewing.
