loading precast
forms, in the yard of the establishment, were exposed to the hazard of
being thrown off the
forklift and crushed due to no seat belt on the TCM forklift, Serial
Number 59259607732,
and an inoperable seat belt on the TCM forklift, Serial Number 43950771.
A feasible and
useful method of abatement, among other methods, is to install and require
the use of the
seat belt by all employees operating forklifts.
