A.  Employer knew or should have known that two employees operating
Komatsu forklift,
Serial Number 516148A, were exposed to the hazard of being thrown from the
forklift and
crushed due to employer not requiring the use of a seat belt in the
establishment.  A feasible
and useful method of abatement, among other methods, is to require the use
of a seat belt by
all employees operating forklifts.
