A.  Employer knew or should have known that foreman operating Caterpillar
loader, Model
416C, without a seat belt worn was exposed to the hazard of being thrown
from the loader at
the Saint Gregory job site.  A feasible and useful method, among other
methods, to correct
this hazard is to ensure operator wears seat belt.
