SCRR 71-112A: Failed to furnish a place of employment which is free of recognized hazards which may cause death or serious physical harm to his employees and comply with this such as, but not limited to, Lull forklift, Serial Number APR96V13N17-415, was exposed to the hazard of being crushed by overhead cage of forklift, in the event of a tip over, while not wearing seat belt on forklift. A feasible and useful means of abatement, among others, is to require and enforce the use of the provided seat belt.
