Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to crushing hazards due to failure of hoists and its'
components in that hoists were
being left suspended and unattended for an extended amount of time and
hoist operators were
not adequately trained on operating procedures for operating hoists such
as the DeMags and
Yales in the following areas:
(a)In the Axle area, operators left loads suspended on the Yale hoists for
extended periods
of time and, work under, near and around suspended and moving loads, and
allowed their
attention to be diverted during lifts, in that hoist operators were
performing duties such as
placing a piece of wood in the frame of the axle while operating the hoist.
(b)In the Turnover area, operators operating the two Yale 5-ton turnover
hoists
were not
aware of the weight of the load (chassis) lifted on the turnover hoists,
and allowed their attention
to be diverted when turning over the load in that hoist operators would
use their foot to move
the dollies into place and hold them in place while operating the hoists.
(c)In the Cab Set area, operators operating the DeMag 2-ton hoists were
working under
suspended loads and working under loads (cabs) while they were still
moving such as the cabs,
left loads suspended (staging area) for an extended amount of time and had
their attention
diverted when operating the hoists in that operators would also move hoses
while operating the
hoists.
(d)In the Engine Set area, operators operating the Yale 3-ton hoists left
the load suspended
and worked under and around the suspended loads (motors) and had their
attention diverted
while lifting loads in that the hoist operators' would move hoses under
the load while operating
the hoist.
(e)In the Radiator Set area, operators operating the Yale hoists left the
load suspended and
worked under and around the suspended loads (radiators).
(f)In the Staging area for cabs the control devices for the DeMag hoists
were left unattended
and the loads were left suspended for an extended amount of time. In the
Axle Set area, Engine
Set area and the Radiator Set area, loads were left suspended by chains
for extended amounts
of time.
Among feasible abatement methods is to follow the recommendations of
ANSI/ASME B30.16-
2003 and the manufacturer's of the Yale and DeMag hoist in regard to the
operating instructions
specified in the hoist manufacturer's manuals.urer's
