Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to a falling/crushing hazard:
a)  Intersection Indian Road & Longwood Lake Road, Jefferson Township
Route:
Employees performing lifting operations were exposed to the hazard of
being struck, and or
crushed by, a moving Mack Truck with a McNeilus rear loading compactor,
while serving as
a spotter for drivers while riding on the step while truck is backing up.
Violation occured on or about 10/11/2006.
b)  Garbage and Recycling Routes (including Chester, Byram and Jefferson
Township
Routes:
Employees performing lifting operations were exposed to hazard of falling
and/or being crushed,
while standing on the riding step of a Mack truck with McNeilus body, and
other Packer trucks,
while being operated at speeds in excess of 10 mph and when distance
traveled between stops
exceeded 2/10 of a mile.
Violations disclosed on or about 10/16/2006.
"Among other methods, one feasible and acceptable abatement method to
correct this hazard is
to review and train all drivers and lifters according to the National
Solid Waste Management
Association (NSWMA) Manual of Recommended Safety Practices RSP00001-2001
for backing
safely, acting as a spotter or lifter" and review and train all drivers
according to review and train
all drivers and lifters according to ANSI Z245.1-1999 Section 6 which
specifies safety
requirements for Mobile Wastes and Recyclable Materials Collection,
Transportation, and
Compaction Equipment.
ABATEMENT VERIFICATION IS REQUIRED within 10 days after the abatement
date. The
certification shall include a statement that abatement is complete, date
and method of abatement,
and states employees were informed of this abatement. Abatement
documentation shall include
documents demonstrating that abatement is complete, such as evidence of
the purchase of
equipment, photographic evidence of abatement or other written records.
