Stat. 182.653 subd. 2: The employer did not furnish to each employee, conditions of employment and a place of employment free from recognized hazards which caused or were likely to cause death or serious injury to employees: An employee was working from an elevated platform attached to a rough terrain forklift without using a personal fall arrest system. The employee was exposed to fall hazards of approximately 10 to 13 feet above grade.
