Nevada Revised Statute 618.375(1):  The employer did not furnish
employment and a place
of employment which was free from recognized hazards that were causing or
likely to cause
death or serious physical harm to employees:
The employer failed to ensure that the operator's manual was followed:
a.An employee performing maintenance on a Case 1840 Uni-loader, attempted
to put
the oil cap back into position with the engine running. The fan blade
amputated the
middle finger on his right hand and fractured the thumb and index finger
on his right
hand.

operation of the machine before you start servicing".

danger decals
in place.
