employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to:
(a)On or about November 28, 2006 - Metal Department, an employee was
operating
a forklift without utilizing the restraint device.
(b)On or about November 28, 2006 - Laminating Department, an employee was
operating a forklift without utilizing the restraint device.
(c)On or about November 28, 2006 - Shipping Department, an employee was
operating a forklift without utilizing the restraint device.
Among other methods, one feasible and acceptable method to correct this
hazard would be to
ensure that all forklift drivers utilize the restraint devices provided on
the forklifts.
