In the heavy equipment maintenance shop, on or about October 13, 2006, and

dates prior thereto, employee(s) were exposed to struck by hazards created
by the
use of the high pressure hydraulic Porta Power unit and the associated work
pieces being used to install a bearing/bushing.
Feasible methods to abate this hazard include, but are not limited to the
following:
a.Ensure the danger zone/work area is barricaded so no employees
are exposed to a struck by hazard.
b.Ensure the area where the bearing/bushing is being pressed in or
where similar work is being conducted is guarded so no flying
parts could strike an employee.

appurtenances for bearing/bushing insertion into heavy equipment.  The
employer
did not ensure employees were trained in the safe operation of the
hydraulic
equipment as well as the proper selection of associated parts for use with
hi-
pressure hydraulics to prevent a mishap.
Feasible methods to abate this hazard include,but are not limited to the
following:
The employer must ensure that all operators are trained in the hazards
associated
with the safe operation of the Porta Power unit as required by ANSI B30.1
and
the manufacturer's operating instructions for hydraulic cylinders.
