Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to caught between and struck by hazards: (a)Reliance Precast LLC, at 4558 Weld County Road 12, Dacono, CO: On 10/20/06, Reliance Precast LLC did not ensure that an effective operator training program was developed and implemented for operation of rough terrain forklifts. Employees operated Lull and Gradall rough terrain fork lifts to transport troughs of ready mix concrete and transport precast concrete blocks. The employer did not develop and implement an operator training and evaluation program. This condition exposed employees to caught between and struck-by hazards. Abatement Note: Among other methods, one feasible and acceptable abatement method to correct this hazard is to provide a training program which complies with the requirements of Section 5.17.4 of ASME B56.6-2002, The American Society of Mechanical Engineers, An American National Standard, "Safety Standard for Rough Terrain Forklift Trucks".
