employer did not
furnish employment and a place of employment which were free from
recognized hazards
that were causing or likely to cause death or serious physical harm to
employees, in that
employees were exposed to struck-by hazards due to the inadvertent release
of pressurized
natural gas:
(a)Schneider Energy Services, Inc. & Webb 11-04 NESW Sec.4 T10S R95W,
Collbran, CO 81624: On or before November 28, 2006, the employer did not
ensure that employees performing a "cold cut and cap" operation involving a
4" pipeline were protected from struck-by hazards associated with
pressurized
natural gas. The employer did not have sufficient procedures and controls
implemented to effectively ensure the pipeline was de-energized before
employees initiated the process of cutting into the pipe.
Abatement Note: Among other methods, one feasible and acceptable method to
correct this
hazard is to develop and implement specific procedural elements to control
hazardous energy
in accordance with the American National Standards Institute
(ANSI)-American Society of
Safety Engineers (ASSE) Z244.1-2003 publication.
