Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish a place of employment which was free from recognized hazards that
were causing or
likely to cause death or serious physical harm to employees in that
employees were exposed
to struck by hazards:
(a)Frank's Westates Services Inc. at Petro-Hunter - Bronco Rig 23,
Parachute, CO
81635:  On or before 4/19/07, the employer did not ensure that the base
mounted
winches on the three trucks based out of the Grand Junction office were
marked with
the following identification for each drum:
a)load rating;
b)drum size consisting of barrel diameter, barrel length, and flange
diameter;
c)rope size(s);
d)rope speed in feet per minute (meters per second); and
e)rated power supply.
Condition exposed employees to struck by hazards.
Abatement Note:  Abatement certification and documentation are required
for this item (See
enclosed "Sample Abatement-Certification Letter").
Among other methods, one feasible and acceptable abatement method can be
found in the
American National Standards Institute B30.7-2001 Section 7-1.1.3, as
referenced, in part,
above.
Abatement Note:  Please note that a resolution to this citation may
require pull testing the
winches, and ensuring the ratings include an appropriate safety factor.

likely to cause death or serious physical harm to employees in that
employees were exposed
to struck by hazards:
(a)Frank's Westates Services Inc. at Petro-Hunter - Bronco Rig 23,
Parachute, CO
81635:  On or before 4/19/07, the employer did not ensure that each
section of high
pressure hoses was secured to prevent movement should the line fail.
These hoses
provide up to 4,000 pounds of hydraulic pressure, and are connected with
hammer
unions.  While proper connections are required to allow the diaphragms
within the
connections work, there was evidence of corrosion that could prevent the
diaphragms
from working properly.  A separation of this connection would result in
the ends of
the hydraulic hoses whipping around.  Condition exposed employees to
struck
by
hazards.
Abatement Note:  Abatement certification and documentation are required
for this item (See
enclosed "Sample Abatement-Certification Letter").
Among other methods, one feasible and acceptable abatement method can be
found in API
Recommended Practice 54, Section 9.13.3, which provides that, "Each
section of high
pressure rigid line should be secured using appropriate means to prevent
movement should
the line fail.  Flexible high pressure hose should be properly secured to
prevent unsafe
movement."
Abatement Note:  It is important to recognize that not all safety chains
are approved for high
pressure hydraulic hoses.  Some are approved only for lower pressure
pneumatic
connections.  When purchasing safety clamps, please make sure that the
items purchased are
approved by the manufacturer for high pressure hydraulic hoses.
