Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to "struck by" hazard:
a)  Grey Wolf Drilling Company, Rig 708, Collbran, CO: On and prior to
5/2/2007, the off drillers side air hoist used to raise and lower rig
components
and drill pipe was operating with a faulty main control lever.  The main
control
lever was not capable of returning to the center or neutral position when
released.
Among other methods, one feasible and acceptable abatement method to
correct
this hazard is to follow the manufacturers operating practices contained in
publication form P5869, Edition 8, dated February, 1981. These operating
practices include inspecting the winch at the beginning of each shift to
ensure that
controls function properly and return to neutral when released.

were exposed to blunt force trauma:
a)   Grey Wolf Drilling Company, Rig 708, Collbran, CO:  The employer had
not ensured that the auxiliary escape line route from the derrick was
clear of
obstructions in the landing zone. On and prior to 5/2/2007, various
drilling
equipment including but not limited to a down hole mud motor and drill
collars
were placed in the landing zone.
Among other methods, one feasible and acceptable method to correct this
hazard
is to keep the auxiliary escape line route clear of obstructions in
accordance with
the Recommended Practice for Occupational Safety for Oil and Gas Well
Drilling
and Servicing Operations API 54, section 6.10.1.
