Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that the employer did not ensure proper documentation of category III inspections of rig #12: (a)Cyclone Drilling Inc., Rig 12, Rifle, CO 81650: On August 31, 2007 the employer did not ensure proper inspection documentation of Rig 12 in that the forms as presented in API-4G were not used. This condition exposed the employees to hazards of working on a rig with the potential of unsafe conditions. Abatement Note: Abatement certification is required for this item (See enclosed "Sample Abatement-Certification Letter").
