recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to the following:
a.  On the site, the employer did not ensure the safety pallet used to
lift personnel to the roof
area was provided with a guardrail system.
Among the feasible means of abatement that can be used to correct this are:
Utilize only platforms designed for use in elevating personnel with
forklifts.  These must
meet the requirement set forth in ANSI/ASME B56.6-2002, section 8.24.1(l),
in that a
restraining means such as a guradrail or a means for securing personnel
such as a body
belt or lanyard shall be provided.
