REF: ANSI/ASME B30.11 - 1998 Chapter 11-2 Inspection Testing, and Maintenance; Section 11- 2.2 Testing; Paragraph 11-2.2.2.1 Operational Tests; Sub-Paragraph (c) Altered, repaired, and modified cranes and monorails shall be tested. Note:Among other methods, one feasible and acceptable method to correct this hazard is to have a rated load test performed by a competent person after modifications have been completed and to maintain the documentation of the load test on site in a permanent file for the operational life of the crane.ional

North Carolina General Statute 95-129(1) of the Occupational Safety and Health Act of North Carolina: The employer did not furnish to each of his employees conditions of employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees: a)Planer Department -- an initial rated load test was not conducted on an under the hook lifting device that was used with a 1 ton CM Load Star jib crane used to lift cutting heads weighing up to approximately 800 lbs. REF: ANSI/ASME B30.20-1985 Chapter 20-1; Section 20-1.4 Testing; Paragraph 20-1.4.1 Operational Tests; Sub-Paragraph (a) Prior to initial use all new, altered, modified, or repaired, lifting devices shall be tested to ensure compliance with this standard. Note:Among other methods, one feasible and acceptable method to correct this hazard is to have a rated load test of each crane installed at the facility performed by a competent person and to maintain the documentation of the load tests on site in a permanent file for the operational life of the crane.ional

REF:ANSI/ASME B30.20-1985 Chapter 20-1; Section 20-1.2 Markings and Construction; Paragraph 20-1.2.1 Markings; Sub-Paragraph (a) The rated load of the lifting device shall be marked on the main structure where it is visible. Note:Among other methods, one feasible and acceptable method to correct this hazard is to have a rated load marking placed on the lifting device of the main structure where it will be visible.e
