Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that: a) Workplace: on or about 1/7/08, employees had not received training on operating the vertical reciprocating conveyor. ABATEMENT CERTIFICATION AND DOCUMENTATION IS REQUIRED FOR THIS ITEM. Feasible means of abatement are: 1. Provide operator training to all employees that are required to operate this conveyor. Reference ASME/ANSI B20.1 Section 5.12(a) Operation which state: ONLY TRAINED PERSON SHALL BE PERMITTED TO OPERATE A CONVEYOR. TRAINING SHALL INCLUDE INSTRUCTION IN OPERATING UNDER NORMAL CONDITIONS AND EMERGENCY SITUATIONS.

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that: a) Workplace: on or about 1/7/08, employees operating a vertical reciprocating conveyor were exposed to a 24 foot fall hazard from not maintaining and inspecting the lift on a regular basic as required in the CIP Operators Manual Section IX and ASME/ANSI B20.1 Section 5.12 (h). ABATEMENT CERTIFICATION AND DOCUMENTATION IS REQUIRED FOR THIS ITEM. Feasible means of abatement are: 1. To begin a schedule of inspections every 6 months by an competent person as required in CIP Operators Manual section IX Inspections and Maintenance. Items to inspected: a) Hoist cable b) Guide roller mounting bolts c) Plastic (nylatron) guide rollers d) Slack cable arm & switch e) Platform braking f) Electrical system g) proper spooling of cable on drum
